Modiwake is an oral medication aimed at enhancing wakefulness in individuals experiencing excessive sleepiness. Its effects are akin to those of caffeine, as both substances stimulate the brain to promote alertness, categorizing them as stimulants. Other stimulants, such as dextroamphetamine and sibutramine, additionally have the effect of appetite suppression. Although all these medications influence neurotransmitters in the brain—chemicals that facilitate communication between nerve cells—they interact with these neurotransmitters in distinct ways.
Modiwake is indicated for enhancing wakefulness in patients suffering from excessive sleepiness related to narcolepsy, obstructive sleep apnea/hypopnea syndrome (as an adjunctive treatment for the underlying obstruction), and shiftwork sleep disorder. Narcolepsy is a chronic condition affecting the brain and spinal cord, often marked by an irresistible urge to sleep. Shiftwork sleep disorder results from disruptions to the normal sleep-wake cycle, frequently due to nighttime work.
Typically, Modiwake is administered at a daily dose of 200 or 400 mg, though the 400 mg dosage has not demonstrated improved efficacy over the 200 mg dosage. Modiwake can be taken either with or without food.
Modiwake can either enhance or diminish the activity of liver enzymes that metabolize (eliminate) other medications. This may lead to reduced concentrations of certain drugs, diminishing their effectiveness, while increasing the levels of others, potentially causing toxicity. Therefore, careful monitoring of Modiwake treatment is essential when taken alongside these medications.
Drugs that might exhibit decreased effectiveness when used with Modafinil include cyclosporine, theophylline, and hormonal contraceptives such as progestin-only or those containing estrogen and progesterone. It is advisable to consider alternative hormonal contraceptives or an additional non-drug contraceptive method during Modiwake treatment, which should extend for one month after discontinuing Modiwake therapy, due to the lingering effects of Modafinil on contraceptive hormones that may last for several weeks. The impact of Modiwake on hormonal contraceptives can occur with oral, patch, or implant forms of hormonal intake. The interaction between Modafinil and alcohol remains uncertain.
The most frequently reported side effects of Modiwake observed in clinical trials include headache, upper respiratory tract infection, nausea, nervousness, anxiety, and insomnia, each occurring in over 5% of patients.
